In addition, the Website provides general information to members and non-members on topics relevant to the content of TellThem4Me® and our related services.

Each section of the members Website allows you to enter as much or as little information as you choose to add. TellThem4Me® takes no responsibility for the choices you make with regards to the information you choose to record.

B) Personal information you may choose to submit when you create a free ICE Card or ICE File (with or without purchasing a TellThem4Me® membership):
- First, middle, and last name
- Emergency contact – name(s), relationship to you and contact number
- Essential health and medical information including medical conditions, allergies, medications, and other notes
- First, middle, and last name
- Date of birth
- Email address
- My Emergency contact – name(s), relationship to you, contact number, medical conditions, allergies, medications, and notes
- My Dependents – names and care arrangements
- My Estate – Last Will & Testament date and location, details, main bank name, main bank account details (including branch and account number)
- My Digital Footprint – main website addresses, mobile apps, usernames, and location / password (N.B. Passwords to eb added in writing once your ICE File is printed)
- My Health Care – GP & healthcare provider details, health conditions, medications, allergies, Advanced Health Care Directives, organ donation, DNR instructions, Ambulance cover and health insurance details
- My Insurance – superannuation, personal accident, accidental death, life, and prepaid funeral insurance details
- My Wishes – funeral provider and funeral service details
C) Personal information you may choose to submit when you sign up for a TellThem4Me® Membership. Personal information you may choose to record in the members only sections* includes:
All About Me* – This section allows you to record personal information including your name, Date of Birth (DOB), physical and email addresses, and key contacts. You can also record the location of important documents, items, and keys. This Privacy Policy also protects sensitive personal information such as information about your religion and ethnicity and by providing this information willingly and freely in the members only section, TellThem4Me® assumes that permission and consent has been provided by you.
My Family* – This section allows you to record who your closest family members are and the care arrangements you wish to see implemented if you are unable to care for any child and / or adult dependents and / or pets.
My Story* – This section allows you to record an in-depth timeline of your life including (but not limited to) information about you, your birth, childhood and adult life, qualifications, places of work and any military service you have completed. You can also record your favourites, your love story and leave personal messages for your loved ones.
My Health Care* – This section allows you to record the details of key health care providers and a personal and family health history. You can also record your wishes with regards to your advanced health care and organ donation. The final section is reserved for end-of-life planning and care.
My Insurance* – This section allows you to record details of key insurance policies including the type, provider details and policy numbers. You can add extra notes if you wish.
My Digital Footprint* – This section allows you to leave records of websites and apps that you use along with usernames to make life much easier for yourself and your loved ones. For security reasons you should not store your passwords online. The online component of My Digital Footprint* compliments the free paper-based password book that you will receive as part of your membership pack where you may choose to record your passwords in writing.
My Estate* – This section allows you to record information about your estate, your assets, liabilities, and personal possessions.
My Wishes* – This section allows you to detail your wishes for the type of funeral you would prefer, details about the service and the finer details asked of your loved ones when arranging a funeral.
My Nominees* – This section allows you to nominate up to 3 people to represent you for a time when you can’t communicate yourself. You will need to add their name, contact details including email address and contact phone number. You have full control over who your Nominees are and the section(s) they can view. Nominees have no editing rights. TellThem4Me® will assume that permission and consent has been provided by your Nominees and yourself.

D) Personal information you choose to submit when you participate in any surveys.
TellThem4Me® collects information via unpaid and voluntary online surveys. We will use information collected through such surveys for the purposes described in this Privacy Policy.
